    Influence of the Flexible Liposomes on the Skin Deposition of a Hydrophilic Model Drug, Carboxyfluorescein: Dependency on Their Composition

    This study focuses on the effect of different flexible liposomes containing sodium cholate, Tween 80, or cineol on skin deposition of carboxyfluorescein (CF). Size distribution, morphology, zeta potential, and stability of the prepared vesicles were evaluated. The influence of these systems on the skin deposition of CF utilizing rat skin as membrane model was investigated. Results showed that all of the investigated liposomes had almost spherical shapes with low polydispersity (PDI < 0.3) and particles size range from 83 to 175 nm. All liposomal formulations exhibited negative zeta potential, good drug entrapment efficiency, and stability. In vitro skin deposition data showed that flexible liposomes gave significant deposition of CF on the skin compared to conventional liposomes and drug solutions. This study revealed that flexible liposomes, containing cineole, were able to deliver higher amount of CF suggesting that the hydrophilic drugs delivery to the skin was strictly correlated to the vesicle composition

    Intellectual Framework for Transforming Conventional Industrial Zones into Eco- Industrial Parks

    Industrial practice is vital to achieving economic growth and prosperity in both developed and developing countries - of which Egypt is one. However, it harms the environment by high rates of consumption, depletion of natural resources and disposal of waste. Therefore, transformation of the conventional industrial system to be compatible with the ecosystem and its capabilities to renew itself is necessary. This approach requires minimization of waste, extraction of raw materials and use of nonrenewable resources. This can be accomplished through Industrial Ecology (IE) and Industrial Symbiosis (IS), which maximize utilization of resources in the production process by using waste as a valuable “by-product” rather than disposing it in landfills. Hence, based on normative and experimental literature in IE, IS and previous eco-industrial parks (EIP) experiences, this research attempts to provide an intellectual framework for the planning process for the transformation of conventional industrial zones (CIZs) into EIPs. It is composed of five stages: selection of industrial zones, local context analysis, planning eco-industrial park (EIP), community participation & raising awareness and organizational structure. we hope that this research will represent a reference starting point in the field of industrial ecology planning for urban planners, practitioners and academics and to be a contribution to the scientific debate related to transforming CIZs to EIPs

    Comparative study between purse–string suture and peritoneal disconnection with ligation techniques in the laparoscopic repair of inguinal hernia in infants and children

    Background Laparoscopic hernia repair in children is becoming more popular nowadays. A lot of laparoscopic techniques were described to repair inguinal hernia in infants and children; however, there are few reports on laparoscopic disconnection of the hernia sac at internal inguinal ring (IIR) as a method for hernia repair.Purpose The objective of this study was to compare intracorporeal purse–string suturing leaving the hernia sac in continuity and laparoscopic disconnection of the hernia sac at IIR and proximal closure of the peritoneum for repair of inguinal hernia in infants and children. A randomized prospective study was carried out in the Pediatric Surgery Unit of Alexandria University Hospitals (Alexandria, Egypt) on 40 male children.Patients and methods Forty male patients (48 repairs) were randomized into two equal groups (n= 20). Group A was subjected to intracorporeal purse–string suturing around the IIR leaving the hernia sac in continuity. Group B was subjected to disconnection of the hernia sac from the parietal peritoneum at the level of IIR, followed by proximal closure of the peritoneum. Inclusion criteria were as follows: male inguinal hernia, either unilateral or bilateral, and age between 6 months and 12 years. Exclusion criteria were as follows: female inguinal hernia, hernia with undescended testicles, recurrent inguinal hernia, and previous major lower abdominal surgery. The main outcome measurement was recurrence, and secondary outcome measurements were operative time, hospital stay, intraoperative complications, postoperative hematoma, postoperative testicular atrophy, and postoperative hydrocele formation.Results There were no significant differences between the two groups as regards age, sex, and mode of presentation. All cases were completed laparoscopically without conversion. Group A showed a significantly higher rate of recurrence as well as hydrocele formation compared with group B; however, there was no difference as regards the operative time, hospital stay, and testicular atrophy.Conclusion Laparoscopic hernia repair using the peritoneal closure following disconnection of the hernia sac is a safe and feasible method for hernial repair with minimal complications. It has a lower recurrence rate compared with the purse–string suturing alone, with no added risk for injury to the vas and vessels

    Neutrophil-to-lymphocyte ratio [NLR] as a promising prognostic marker in critically Ill septic patients

    Background: NLR is advantageous in regard to simplicity, low cost, and availability compared to many other previously proposed biomarkers, which makes it promising for diagnostic clinicians. Several studies have reported that the NLR is useful in various clinical situations.Objective: To determine whether NLR obtained from complete blood count (CBC) and with simple calculation can be used to predict mortality in patients with sepsis and septic shock in the ICU in comparison with intensive care unit (ICU) severity scores.Patients and methods: This prospective trial was carried out on 84 ICU cases with severe sepsis, who were admitted to Specialized Medical Hospital ICUs from June 2020 to June 2021.Results: There was statistically significantly higher systolic blood pressure (mmHg), mean arterial pressure, rate pressure product *10^3, EF, INR, RBS, PH, HCO3, L *10^3, platelet count, CRP on admission, ABACHEII score, SOFA score on admission, duration of hospital stay (days), and a statistically significantly lower age (years), diastolic blood pressure (mmHg), heart rate, respiratory rate (RR), GCS, serum creatinine (mg/dl), serum albumin, serum bilirubin, Na, K, total leucocytic count *10^3, N *10^3, in group B NLR &gt;10 vs. group A NLR≤ 10.Conclusion: Neutrophil to lymphocyte ratio is a cheap and rapidly available predictor of sepsis and has shown a significant correlation with other relatively expensive and non-rapidly existing markers of inflammation and sepsis with comparable efficacy with ICU severity scores [SOFA and APACHE II]

    Garlic Increases Antioxidant Levels in Diabetic and Hypertensive Rats Determined by a Modified Peroxidase Method

    Oxidative damage by free radicals has been implicated in the pathogenesis of vascular disease in diabetes and hypertension. In the present study, the total antioxidant status in diabetic and hypertensive rats before and after treatment with garlic (Allium sativum) was determined. The total serum antioxidants were measured by a modified method reported earlier by Miller and coworkers. The reproducibility of the assay was confirmed by determining standard curves for the known antioxidants: trolox (a stable analog of vitamin E), glutathione and vitamin C with interassay correlation coefficients (R2, n = 10 in triplicate) of 0.9984, 0.9768 and 0.987, respectively, confirming the reliability and reproducibility of the assay. This assay was then used to determine total serum antioxidant levels of streptozotocin-induced diabetic rats and two-kidney one-clip hypertensive rats both before and after 3 weeks of treatment with an aqueous extract of garlic (500 mg/kg IP daily). The serum antioxidant levels of rats after 3 weeks of treatment were significantly higher (P < .001) than the pretreatment levels in both diabetic and hypertensive rats. The increased serum antioxidant levels were paralleled by a decrease in serum glucose in the garlic-treated diabetic rats and lowered systolic blood pressure in the garlic-treated hypertensive rats. We conclude from our study that (i) total antioxidants can be measured by a simple, reproducible, reliable assay and (ii) the total antioxidant status can be significantly improved by treatment with garlic

    Bilosomes as a promising nanoplatform for oral delivery of an alkaloid nutraceutical:improved pharmacokinetic profile and snowballed hypoglycemic effect in diabetic rats

    Diabetes mellitus is a life-threatening metabolic disease. At the moment, there is no effective treatment available to combat it. In this study, we aimed to develop berberine-loaded bilosomes (BER-BLS) to boost the oral bioavailability and therapeutic efficacy of berberine, a natural antidiabetic medication. The BER-BLS was fabricated using a thin-film hydration strategy and optimized using a central composite design (face-centered). The average vesicle size, entrapment efficiency, and surface charge of the optimized BER-BLS preparation were 196.5 nm, 89.7%, (−) 36.4 mV, respectively. In addition, it exhibited higher stability and better-sustained release of berberine than the berberine solution (BER-SOL). BER-BLS and BER-SOL were administered to streptozocin-induced diabetic rats. The optimized BER-BLS formulation had a significant hypoglycemic impact, with a maximum blood glucose decrease of 41%, whereas BER-SOL only reduced blood glucose by 19%. Furthermore, the pharmacological effect of oral BER-BLS and BER-SOL corresponded to 99.3% and 31.7%, respectively, when compared to subcutaneous insulin (1 IU). A pharmacokinetic analysis found a 6.4-fold rise in the relative bioavailability of berberine in BER-BLS when compared to BER-SOL at a dosage of 100 mg/kg body weight. Histopathological investigation revealed that BER-BLS is suitable for oral administration. Our data demonstrate that BLS is a potential nanocarrier for berberine administration, enhancing its oral bioavailability and antidiabetic activity

    Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis

    BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration.     Prevalence of trachoma in four marakez of Elmenia and Bani Suef Governorates, Egypt.

    PURPOSE: In 2015, to determine where interventions are needed to eliminate trachoma as a public health problem from Egypt, we initiated population-based prevalence surveys using the Global Trachoma Mapping Project platform in four suspected-endemic marakez (districts; singular: markaz) of the governorates of Elmenia and Bani Suef. METHODS: In each markaz, 30 households were selected in each of 25 villages. Certified graders examined a total of 3682 children aged 1-9 years in 2993 households, noting the presence or absence of trachomatous inflammation-follicular (TF) and trachomatous inflammation-intense (TI) in each eye. A total of 5582 adults aged ≥15 years living in the same households were examined for trachomatous trichiasis (TT). Household-level access to water and sanitation was recorded. RESULTS: Three of four marakez had age-adjusted TF prevalence estimates in 1-9-year olds of >10%; the other markaz had a TF prevalence estimate of 5-9.9%. Estimates of the age- and gender-adjusted prevalence of unmanaged TT in adults ranged from 0.7% to 2.3%. Household-level access to water and sanitation was high. (We did not, however, measure use of water or sanitation facilities.) Conclusions: Each of the four marakez surveyed has trachoma as a public health problem, with a need for implementation of the SAFE (surgery, antibiotics, facial cleanliness, environmental improvement) strategy. Further mapping is also required to determine the need for interventions in other areas of Egypt
